ElderCare Assessment

The ElderCare Assessmentis available strictly through ElderCare at Home & the ElderCare Resource Center.  It is the most duplicated and sought-after assessment tool utilized in the industry today. 

There are many reasons that you might need or want an ElderCare Assessment performed:

  Your loved one has been given a probable diagnosis of Alzheimer's disease, dementia or suffers from another memory impairment 

  There are concerns regarding medication and/or compliance with Physician's orders.

  Your loved one lives alone.

  The primary caregiver is frail and could benefit from professional guidance and support.

  The patient has multiple medical problems.

  The patient has suffered a recent physical or psychological trauma such as a hospitalization or the death of a loved one.

   You have noticed a change in either mental or physical status.

   There is not a strong, local support system in place.

   You think your loved one is depressed or they no longer participate in activities that once brought them joy.

  There are concerns regarding driving ability.

  You are not sure if your loved one is eating or preparing nutritious meals.

    You live at a distance and just want some peace of mind.

The ElderCare Assessment may include an evaluation of the following: 


1.     Cognitive and functional strengths and limitation

2.     Finances, insurance policies, estate planning

3.     Home safety evaluation and fall risk assessment

4.   Speech and oral expression skills

5.     Pain assessment

6.     Emotional, cognitive, behavioral, social, spiritual status

7.     Activities of Daily Living (ADL's)

8.   Instrumental Activities of Daily Living (IADL's)

9.   Depression screening

10.  Mini-Mental Status Exam

11.  Caregiver Burden assessment

12.  Supportive assistance review

13.  Medical records review

14.  Medication review

15.  Telephone consultation with family members
